If you are paying by Credit Card, please fill the Credit Card Authorization Form and Fax: it to the 305-531-1165 together with a photocopy of a Picture ID and the Front and Back of the card used.

WE DO NOT PROCESS ONLINE PAYMENTS OTHER THAN PAYPAL

Credit Card Payment are manually processed over the phone. No credit card payment will be processsed until the Credit Card Authorization Form is received.
